What is back pain?

Back pain may occur in the upper, middle or lower back. Lower back pain is the most common and is sometimes referred to as lumbago.

The symptoms may develop after heavy lifting, unfamiliar movements or prolonged strain. In many cases, it is not possible to identify one specific cause of the pain.

Common symptoms of back pain

Back pain may cause:
Aching or sharp pain
Back stiffness
Pain during movement
Muscle tension
Reduced mobility
Pain radiating to the buttock or leg
Tingling or numbness
Difficulty sitting, standing or walking
Symptoms vary depending on the location and cause of the pain.

Common causes of back pain

Back pain may be caused by:
Muscle strain or overuse
Heavy lifting or incorrect lifting technique
Unfamiliar physical activity
Prolonged sitting or an unfavourable working position
Wear and tear in the joints of the spine
A slipped disc or nerve irritation
A fall or another injury
In some cases, back pain may be caused by a condition elsewhere in the body. The doctor will therefore assess your symptoms and whether further examinations are needed.

When should you contact a doctor?

Contact a doctor if:
The pain is severe or becoming worse
The pain does not improve as expected
The pain radiates down one or both legs
You have tingling, numbness or weakness in the leg
The back pain started after a fall or injury
You have a fever or feel generally unwell
The pain is worst at night or while resting
You have cancer, osteoporosis or a weakened immune system

Severe back symptoms

Seek urgent medical help if you develop:
New weakness or paralysis in the legs
Numbness in the groin or buttock area
Loss of bladder or bowel control
Difficulty urinating
Severe pain after a serious accident
Back pain together with feeling severely unwell
These symptoms may indicate serious nerve involvement and require immediate assessment.

Contact the public out-of-hours medical service on 116 117, or call 113 if the condition is severe or worsening rapidly.

Examination and treatment

The doctor will ask when the pain started, where it is located, whether it radiates and which movements affect it.

If necessary, the doctor may:
Examine your back and mobility
Assess muscle strength and reflexes
Assess sensation in the legs
Assess whether pain relief is needed
Assess whether you need a sick note
Refer you for physiotherapy, diagnostic imaging or further investigation
Most cases of acute back pain improve gradually. It is usually recommended to remain gently active and avoid prolonged sitting or bed rest.
